Brief specification description or basic overview of the project:Purchase physical examination services for specific populations in Nanhui New Town, including "Ivy" elderly health examinations and "Carnation" gynecological examinations. The winning bidder shall meet the relevant physical examination and information management needs of Party A, and share the physical examination data with the community health service center in this town.
合同履約期限：本項目一招三年，合同一年一簽。首年服務(wù)期為自合同簽訂起至****年**月**日。付款金額以當年度實際體檢人數(shù)為準。
The Contract Period:This project has a three-year contract with a yearly signing. The first year of service period is from the signing of the contract to October 1, ****. The payment amount shall be based on the actual number of physical examination personnel in the current year.

(b)落實政府采購政策需滿足的資格要求：（1）落實預留份額措施，提高小微企業(yè)在政府采購中的份額，扶持小微企業(yè)政策：本項目（□是 ■不是）專門面向中小微企業(yè)采購，評審時小微企業(yè)價格給予**%的扣除。（2）扶持殘疾人福利性單位，并將其視同小微型企業(yè)；

(c)本項目的特定資格要求：（1）本項目面向大、中、小、微型企業(yè)，事業(yè)法人、其他組織或自然人采購。
（2）法人依法設(shè)立的分支機構(gòu)以自己的名義參與投標時，應提供依法登記的相關(guān)證明材料和由法人出具的授權(quán)其分支機構(gòu)在其經(jīng)營范圍內(nèi)參加政府采購活動并承擔全部民事責任的書面授權(quán)。法人與其分支機構(gòu)不得同時參與同一項目的采購活動。
（3）具有中華人民共和國醫(yī)療機構(gòu)執(zhí)業(yè)許可證。
（4）符合《關(guān)于對接國際高標準經(jīng)貿(mào)規(guī)則推進試點地區(qū)政府采購改革的指導意見》（滬財采〔****〕**號）第**條規(guī)定的供應商，不得參加本項目的采購活動。
（5）本項目不允許轉(zhuǎn)包。
(c)Specific qualification requirements for this program:(1) This project is aimed at large, medium, small, and micro enterprises, public institutions, other organizations, or natural persons for procurement. (2) When a branch established by a legal entity participates in bidding in its own name, it shall provide relevant certification materials registered in accordance with the law and a written authorization issued by the legal entity authorizing its branch to participate in government procurement activities within its business scope and bear all civil liabilities. (3) Having a practicing license for medical institutions in the People's Republic of China. (4) Suppliers who comply with Article ** of the Guiding Opinions on Promoting Government Procurement Reform in Pilot Areas in Connection with International High Standard Economic and Trade Rules (Hu Cai Cai Cai [****] No. **) are not allowed to participate in the procurement activities of this project. (5) This project does not allow subcontracting.
